Download Immunization Records

If you have submitted or received immunization at UHS and have access to your My Health Beacon Portal, you can easily access and download/print your immunization records:

1. Log in to your My Health Beacon Portal.
2. Click on the "Immunizations" tab. 
3. Click "Print" on the top right-hand side. This will display all your immunizations with dates, ready for printing. You may be able to change your printer to "Save as PDF" to download your records.

Request Medical Records

To have UHS release your medical information to another provider or if you need copies of your medical and immunization records, follow these steps:

1. Download and complete the Release of Medical Information Form
2. Submit the completed form through one of the following methods:
   1. Email: UHS@umb.edu. 
   2. Fax: 617-287-3977 (Attn: Medical Release)
   3. Mail: UHS General Medicine, 100 Morrissey Blvd., Boston, MA 02125 (Attn: Medical Release)
